The intra-operative photo (B) depicts a fibrotic band (straight line) across the anterior aspect of the median nerve (arrow). Arrowheads = arteries, * = pronator teres muscle. The ultrasound image was obtained with a Philips iU22 scanner (Philips Medical Systems, Bothell, WA) with a 12 MHz linear array transducer.
